More About the Dental Plan
The dental benefit is split into 4 categories: Preventive, Basic, Major, and Orthodontia.
Preventive:
Services like x-rays and cleanings are paid for by the plan 100%.
Basic and Major:
Services are subject to an annual deductible. After the deductible the plan pays 80% for basic services and 50% for major services.
Orthodontia:
Services are covered at 50% up to the $1,000 individual lifetime orthodontia maximum.
Basic services are things like emergency care, fillings, extractions, etc... Major services are things like crowns, inlays or onlays, bridgework and dentures, etc.... The Leavitt Group dental plan covers both in-network and out-of-network dentists. The difference is that the in-network dentists are subject to Delta Dental discounts, so your out of pocket costs will likely be lower.
